Routine Appointments

We have a limited number of routine appointments with clinicians you can book through the NHS App.

If there are no appointments available on the app, please call us on 01429 272000 to make an appointment. 

This Practice offers ten-minute appointments for routine matters. We aim to offer you an appointment with a Healthcare professional within 24 hours and a GP within 48 hours, these can also be booked up to four weeks in advance.

Urgent Appointments

If you think you need to be seen urgently, please call the practice on 01429 272000.

You will be added to the telephone triage list.  A clinician will make contact with you by telephone and if deemed necessary a face to face appointment will be arranged. 

Some face-to-face on the day urgent appointments are available. 

If you feel you need to be seen urgently, you must be available to to attend the Practice that day.

 

Evening and Weekend Appointments (Enhanced Access)

You can pre-book appointments around your busy daily schedules and appointments are now available SEVEN days a week.

Hartlepool & Stockton Health (H&SH) offer access in the evening and on weekends on behalf of this Practice.  Appointments are available to see a GP, Nurse or Healthcare Assistant at Chadwick Practice in One Life Hartlepool Building 

Appointments are available: 

Monday - Friday: 18:30-21:00

Saturday & Sunday: 09:00-17:00

They will have access to your notes, and you will see a GP/ Advanced Practitioner/ Nurse Practitioner or Nurse.  Depending on your needs and who is most suitable for your illness/ailment.

Home Visits

Whilst we encourage our patients to visit us in the Practice, where we have the most appropriate equipment and facilities available, we do appreciate this is not always possible. If you do need a home visit, you can help us by contacting the Practice on the dedicated telephone number 01429 287940 before 10:00.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your clinician will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls.

You can also be visited at home by a community nurse if you are referred by your clinician. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

Cancel an appointment

Please make appointments and keep them. If you cannot keep them, please phone us to let us know, a missed appointment is a wasted appointment.

You can also use the NHS App, or SystmOnline to cancel any upcoming appointments.

Contact us using an online form

If your health concern is not urgent, you can use eConsult to get in touch.

Use eConsult to ask us about your health symptoms, conditions or treatment. You can even request things like sick notes and GP letters.

  • Find your symptom, condition or request.
  • Fill out a quick form.
  • The practice responds with advice, a prescription or an appointment.

Patients are also encouraged to check their symptoms, find verified NHS self-help information and find appropriate local or self-referral services, or to submit consultations for their dependents.
