Community staff are no longer attached to Practices in Hartlepool. They are in three locality teams based geographically in the South, Central and Northern areas of the town. The team involved in your care depends on the area in which you live. If you require any of these services you will be referred into the appropriate team by the Practice.
A Health Visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
